A new form dialog has -
	two tabs.
		1. Properties
			which has 3 form elements - 
				a. Name

				c. Class/ID
				
		2. Handlers
		   which has 3 form elements - 
				a. Actions [url]
				b. Method [post, get]
				c. Encoding [text/plain, multipart/form-data, application/x-www-form-urlencoded]
